Medical forms & exams are a necessary part of life for families with school-age children.  To help everyone with meeting State Health Department requirements for attendance in Pennsylvania schools, please note the following mandates.

For ALL NEW and RETURNING BCMCS students:

 

The Emergency Contact Information is available online. For your convenience, Health related forms for  2024-2025 are available by clicking on the "Required Health & Registrations Forms" link in the Nurse's Corner page of our website.  

For returning students leaving 2nd grade to enter 3rd grade:


State Law requires a Private Dental Exam with documentation for entry into 3rd grade. The dental exam form can be found in the "Required Health & Registrations Forms" link to the right of this page. 
Please send in no later than September 2, 2024


For returning students leaving 5th grade to enter 6th grade:


State Law requires a Private Physician's Physical Exam with documentation for entry into 6th grade. The Private Physician Physical Exam Form can be found again in the "Required Health & Registration Forms" link for your convenience. Please send in no later than September 2, 2025

Emergency Contact Information 

 

 New Emergency Contact Information is due EVERY school year or whenever there is a change of information.  Although all of our school forms are important, please  take special care with the Nurse's Office Emergency Information.   

 

Please be sure that contact information is listed in the order you would like called when your child is in need of medications, not already approved to give, or to be picked up for an illness or injury.

Please keep in mind also that for the health & safety of your child along with the other children who visit the Nurse's Office, we ask that those listed are able to pick up your child WITHIN 30 MINUTES.

Calling your Child in Absent


The following procedures have been put in place to assist our teachers in maintaining focus on the children in their classrooms, while streamlining the attendance process. If your child is going to be absent from school please notify the school by 9 am for each day.

Notification can be accomplished by any of the following routes:

Call 215.428.6700 ext. 2 and leave a detailed message at ANY time of day or night, speak with the nurse directly or by email to [email protected].

 

Failure to contact BCMCS may result in your child's absence being marked as illegal or unexcused, if not addressed within 3 days of the absence.


Medication Administration


If your child is on medication that must be given during school hours, please be sure to print and have your physician fill out the Prescription Medication form found in the "Required Health & Registration Forms" link to the left.
